Preparing For Appointment

 

PREPARATION FOR APPOINTMENT:
1.  Divorce without children
      A.  Assets acquired before marriage, by gift or inheritance on both sides.
        B.  Assets acquired during the marriage, including bank accounts and amounts, all personal property valued over $500 (yard sale price), all IRA statements, investment account statements, 401K statements, pension statements, tax returns for the past 3 years, year to date check stubs; deeds of real estate owned by the parties; pending worker's comp or personal injury claim; who should get what and why.
         C.  All debts acquired during the marriage.  List creditor, amount, who should pay what and why.
           D.  Address of spouse
2.  Divorce with Children
      A.  Everything in paragraphs A to D above.
       B.  Names, ages, birthdates of children born to or adopted by the parties, schools, locations and same with after care.  Wishes of children?
        C.  Employer of each party, hours, locations.  Health insurance? Cost per child.  
         D.  Any agreement as to where the children will live when after the divorce/custody case.
          E.  Any agreement on parenting responsibilities: medical, educational, religious, extra-curricular.
             F.  Child care? Cost.
             G.  Extraordinary medical expenses.  Cost.
              H.  Extraordinary extra-curricular expenses.  Cost.
               I.   Extraordinary school expenses.  Cost.
             J.     Address of each party.

 

3.  Criminal.
      Bring copy of all documents received by state.
4.  Probate.  Bring copy of will, if there is one.  List of all major assets and debts. Addresses of spouse, children, grandchildren, parents, siblings of decedent.
5.  Juvenile.  Bring copies of all papers served, petitions, service plans, any other documents.
6.  Adoptions.   
A.  Related or non-Related.  
B.  Agreement or contested.  
C.  Names of Children, ages, birthdates, hospitals born, copies of birth certificates.
D.   Names of adoptive parents
E.   New name for Child?
7.   Advanced directives.
A.  Power of attorney for property, 3 adults
B.  Power of attorney for health, 3 adults
C.  Natural life, extraordinary measures, little hope of quality of life.
D.  Organ donation.
